To chime in on this fantastic discussion...
There is a max innings pitched limit, but no MINIMUM. This is your problem. There are 3 pitching categories to go for here. Wins, saves, holds. Every pitcher has a different purpose. If I go after Holds, then I'm sacrificing something else. Building a team is putting parts together, to achieve all 3 categories.
If it's such a problem, put a minimum limit in place. If you don't hit the minimum, then you get penalized (debatable). As for playing pitchers, and hitters for that matter....I personally like to play the match ups, or chose to play or not play someone based on a gut feeling. Managing is not cut and dry, but about strategy and using history, statistics and your GUT. Sometimes my gut has shit for brains. Sometimes it serves me well.
For teams that are not in the running, why would you not play for the future? The bottom 4-6 teams were pretty close last season. I had the 2 best pitchers (arguably) and sometimes their stats helped, sometimes not.
How someone can complain about a team not playing Matt Carpenter for the last month of the season, take a long look at his stats. They meant nothing except for average and runs. Not missing much there...a team may need other categories instead. Balance is key, but after a point, it is irrelevant for that season. The last place teams stat's generally (I've hardly ever seen it in ROTO leagues) have ZERO effect on the top 3 teams.
